Appointment Request

We see patients Monday through Friday beginning at 8:00 am. To make an appointment to see Dr. Berger, please either call us at 813-265-6940, or fill out the email appointment request below.

If you call, our operator will ask you for a description of your eye problem, your name, address, and phone number and your insurance information.

To request an appointment through email, please fill out the short form below.  Bay Area Eye Institute cannot guarantee the security of the information you submit using this email form.
